Apparently he would impale someone through the anus and out of the right shoulder, by passing vital organs and then sealing the wounds so the person would live for 2 or 3 days in extreme pain, though torture was practised throughout the world at this time. I postulate that Vlad became famous because rulers could say "Sure we drag people out of their homes in the middle of their night and burn them alive, but it could be worse, so be thankful for our rule and don't rebel!".
